Adult _____ Child (0-22) _____ Type of Disability (Check all that apply): Blind / Vision Impaired ? Deaf / Hard of Hearing ? Speech Impaired ? Physically Disabled ? Brain Injury ? Other (Autism, Aspergers Syndrome, Chronic Medical, etc.) Describe: __________________________________________________________ __________________________________________________________ Please prioritize your top three perceived areas of need in the Middle Peninsula: ___ 1. Assistive Technology is any item, piece of equipment, or product system that is used to increase, maintain, or improve functional capacities of individuals with disabilities. Some key areas for considering assistive technology include: augmentative communication, mobility/seating, leisure and recreation, computers, and job site modifications. ___ 2. Case Management is a dynamic collaborative process which utilizes and builds on the strengths and resources of consumers to assist them in identifying their needs, accessing and coordinating services, and achieving their goals. It includes major collaborative components of case advocacy, assessment, planning, facilitation and monitoring. ___ 3. Counseling is both a service and the context in which needs are assessed and information and other services provided. ___ 4. Education All children with disabilities have the right to a free and appropriate education. Services may include special education, speech and language services, occupational and physical therapy, transportation, provision of assistive technology, and transition services. ___ 5. Employment Services encompasses an array of prevocational and vocational preparation activities, new technologies which allow individuals with severe disabilities to compete in the work place, and work site adaptations. Also included in this category of services are job development, placement, supported and sheltered employment and employer support services to promote hiring and adjustment of persons with disabilities. ___ 6. Family Support Services is a flexible and varied network of solutions and information useful for maintaining a family when one of its members has a disability. Family support is a philosophy that permeates the delivery of all other services to persons with disabilities. ___ 7. Accessible Housing There is a general need for affordable, accessible housing. There is also a need for congregate living options for persons needing assistance and supervision ___ 8. Independent Living Services include information & referral, independent living skills training, peer counseling, advocacy, community education, and a variety of other services designed to assist persons with disabilities to lead independent lifestyles and access their communities. ___ 9. Medical & Therapeutic Services Persons with physical and sensory disabilities are likely to need health care services to treat existing conditions and to prevent future health problems. Some previous problems identified in this category include lack of trained health care providers, limited medical-surgical care for medically indigent children with disabilities, availability, and cost of private health care insurance and the high cost of medical care. ___ 10. Personal Assistance Services are those services which make it possible for individuals with severe physical functional limitations to more fully participate in all aspects of daily living and to access other services and opportunities. Examples include help with daily living such as bathing, communicating, cooking, dressing, eating, housekeeping, and transportation. ___ 11. Training refers to the development of qualified service providers, including educators, medical practitioners, sign language interpreters, in-home caregivers, and rehab engineers. ___ 12. Transportation services provide individuals the means of moving from place to place to satisfy some need. Paratransit services refer to on-demand, non-fixed route transportation in an accessible vehicle. ___ 13. Communication Access includes the removal of communication barriers through the provision of auxiliary aids, alternate formats and interpreting services when necessary and requested to ensure effective communication. ___ 14. Emergency Preparedness is a community?s emergency services ability to handle, plan for, and provide education/training for those with physical/sensory disabilities before, during and after an emergency or natural disaster (hurricane, ice/snow storm, tornado, flood, etc). ___ 15. Make a World of Difference Virginians across the state are cleaning litter from waterways this fall in a volunteer effort that is part of an annual International Coastal Cleanup event coordinated statewide in Virginia by Clean VA Waterways/Longwood University. The Northern Neck is no exception, with two events held September 20 at Belle Isle and Westmoreland State Parks, and a third cleanup scheduled for Saturday, October 4, in Richmond County. Volunteers are still needed for this final Northern Neck cleanup: October 4 from 9:00 am to noon Richmond County, Simonson Landing and Farnham Creek Landing (meet at Simonson Landing at 9:00). Contact Faye Andrashko at Northern Neck Soil & Water Conservation District for information or to register: 804 333-3525, ext 113, or faye.andrashko at va.nacdnet.net . Volunteers will be provided with sunscreen, gloves, refreshments, and a certificate signed by Governor Kaine. Please wear shoes that can get wet and bring a hat. Last year, 5,168 volunteers helped in the statewide Virginia Waterways Cleanup event, picking up 283,798 pounds of litter along 432 miles of coastline. The data collected is used by state, federal, and non-profit agencies to identify frequently littered areas and to develop educational programs. This annual international event is part of the world's largest volunteer effort to collect data on litter and trash in the aquatic environment! Cleanup events are held on the coasts, AND inland, since inland litter often ends up in our Bay and our oceans. Also, Virginia was once again in the "Top Ten" of U.S. states in terms of volunteer participation. The goal is to have more than 5,500 volunteers this year!! Each year tons of trash are cleared from waterways. This litter, consisting of cigarette butts, food and drink containers, plastic bags, marine and fishing debris, and other items doesn't just look unsightly. Animals that ingest these materials or become entangled in them suffer painful deaths. Litter in water blocks sunlight needed for aquatic vegetation. People are injured on broken glass and other materials. Boats can become entangled in marine debris and their intake valves blocked by plastic bags. Come be a part of helping your community. Registration is requested to assure enough supplies and refreshments. Faye Andrashko, BMP Information/Education Coordinator Northern Neck Soil & Water Conservation District P.O.
