JOB TITLE:            Program Assistant -Personal Care Program

REPORTS TO:  Program Manager - Personal Care

 

PRIMARY FUNCTION/JOB PURPOSE:

Under the direction of the program manager, to assist in ensuring that
the service to clients and their families is of the highest quality.
Duties will include assisting with record keeping and preparing reports
in a manner that meets regulatory standards and communicating with aides
and clients as directed. Nonexempt, hourly 

 


ESSENTIAL JOB RESULTS/RESPONSIBILITIES:


Assists in maintaining records

Maintains client charts for required forms and information and maintains
files to meet standards

Manages in-service education records

Maintains client and department files

Assists with client admissions as directed


Coordinates and inputs client data 

Sets up client charts      

Obtains billing authorization of the plan of care

Obtains the patient information from local social services

 

Education:

High School Diploma

Computer skills and knowledge required.
